REAP Confidentiality

REAP staff are highly trained and concerned to protect your confidentiality and privacy. We comply with professional standards and government regulations regarding confidentiality.

Usually, you may contact REAP directly without informing anyone at your workplace. When you contact REAP voluntarily, no one at your employer will be informed of this.

REAP staff will keep your request for services and personal information confidential in compliance with state and federal laws and professional standards. Our confidentiality information is available in the Statement of Understanding form that you will sign before your first REAP visit.
You are encouraged to call and personally discuss any questions on confidentiality at any time.

Your contacts will remain totally confidential unless you explicitly approve the exchange of information, usually in writing. Exceptions to this include situations where REAP becomes concerned about a serious threat of harm to self or others, the abuse or neglect of children or the elderly, or if there is a specific court order.

If you were referred to REAP by your employer for job performance reasons, your REAP counselor will fully discuss issues regarding the limited exchange of information.
Limited information about your use of REAP and whether you follow recommendations will be shared only with your written permission or in accordance with government regulations or professional standards.

REAP staff are familiar with HIPAA procedures. For HIPAA purposes, REAP is not defined as a covered entity, because we do not electronically exchange personal health information. REAP does serve as a Business Associate for HIPAA purposes if the sponsoring organization determines this is appropriate for their EAP program.

Participant Records:

REAP’s records are primarily for the use of REAP staff. When clinically and legally appropriate, REAP staff may be able to confirm your attendance, provide a brief summary of your REAP participation, or provide copies of some information in your record.

REAP is very concerned that information in our records not be used in a negative way for our service participants (for example a legal matter or custody issue). There are restrictions on disclosure of information to participants or third parties in accordance with REAP’s policies, professional standards, and legal considerations.

When someone uses REAP in conjunction with a spouse, significant other, or family member, no information or documentation will be disclosed to either party individually or to third parties unless all participants voluntarily authorize release of information in the REAP record.

If you have a desire or a potential need to have information from your record at REAP, you must contact REAP and submit a request, and also sign REAP’s appropriate release authorization form. Requests will be responded to within a 30 day period. An administrative cost for copying of records may be required as allowed by state guidelines.

REAP’s President/ Director, Karen Untz, must approve all release of information requests.
